Update Medical History
We will update your medical records including the following:

-Medication – changes in medication and their dosages.
-New allergies – update new reactions to food, medication, your environment.
-Lifestyle changes – smoking, alcohol consumption, sexual activity, or if you have an advanced directive.
-Mental health – if you are experiencing anxiety or depression.
-Family history – any new health conditions in your family that may increase your risk of developing the condition.

Assess Current Overall Health & Establish Baselines
Blood tests may be recommended to determine future healthcare decisions to save you money and time later:

-Blood pressure screening to assess how your blood pressure is affecting your arteries.
-Cholesterol screening to see how much cholesterol is in your blood.
-Blood glucose screening to assess how much sugar is in your blood.
-Osteoporosis screening to assess bone density (typically done for women over 65 and men over 70)
-Calculate your Body Mass Index (BMI) to help you maintain your ideal weight.

Whether you need to get current with a booster shot for the Tdap vaccine (tetanus, diphtheria and pertussis), especially if you are pregnant, or your flu shot, we can help. We also recommend the HPV vaccine, pneumonia vaccine (recommended if you are a smoker or are older than age 65), the Shingles vaccine if you are over age 60, and the Hepatitis B vaccine if you have diabetes or a compromised immune system.
